CONSENT TO THE PROCESSING OF PERSONAL DATA FOR THE PURPOSES OF OFFERING GOODS AND SERVICES

Hereby, in accordance with Article 7 of the Regulation of the European Parliament and of the Council (EU) 2016/679 of April 27, 2016, the General Data Protection Regulation (hereinafter referred to as "GDPR"), I grant consent to Louda Auto a.s., Company ID: 46358714, with registered office at No. 166, 290 01 Choťánky, (hereinafter referred to as "the Company"), as the data controller, for the processing of my personal data listed below, for the purpose of offering and informing about the Company's products and services and for including this personal data in the database of applicants for services maintained by the Company.

Personal data as referred to in the previous sentence means: name, surname, email address, and phone number.

I acknowledge that the processing of personal data will occur primarily in the following ways: collecting my above-mentioned personal data, storing it on information carriers, modifying and changing it (in case of changes to my above-mentioned personal data), searching, using, and making it available to authorized employees of the Company for the purpose of offering goods and services, as stated below. I further acknowledge that personal data will be processed in printed form in a non-automated manner and also in electronic form in an automated manner.

I agree that the Company may offer me goods and services provided by it or selected third parties known through direct marketing methods, particularly via email, mail, phone, and/or in person. For these purposes, I grant the Company my explicit consent to transfer my above-mentioned personal data to third parties (marketing agencies conducting marketing and advertising activities for the Company).

Furthermore, in accordance with the provisions of § 7 of Act No. 480/2004 Coll., on certain services of the information society and on the amendment of certain laws (Act on certain services of the information society), as amended, I grant the Company my prior consent to use my email address for the purpose of disseminating commercial communications by electronic means.

I declare that the provided data is accurate and true. I grant this consent for a maximum period of 10 years from the date of its granting. I acknowledge that I can withdraw this consent at any time by written notice delivered to the Company at the Company's delivery address: Choťánky 166, 290 01, or via email sent to GDPR@louda.cz.

I further acknowledge that the Company will retain my personal data only for as long as necessary, for a maximum of 10 years, unless this consent is revoked earlier.

I declare that I am aware of my rights against the Company regarding the processing of my personal data as provided in Articles 15 – 22 of the GDPR, particularly:

• my right to be informed by the Company upon request about the processing of my personal data (the content of the information includes a statement about the purpose of processing personal data; about the personal data or categories of personal data that are the subject of processing, including all available information about their source; about the nature of automated processing in connection with its use for decision-making, if actions or decisions are made based on this processing that affect the rights and legitimate interests of data subjects; about the recipient, or categories of recipients), and to access this personal data,
• the right to rectify or change any inaccurate, untrue, or incomplete personal data,
• the right to erasure of personal data, if the conditions under Article 17 of the GDPR are met,
• the right to restrict processing,
• the right to data portability,
• the right to lodge a complaint with the Office for Personal Data Protection or the supervisory authority of another EU member state, and
• the right not to be subject to any decision based solely on automated processing, including profiling.
